The Rare Shotgun Blockhead is manufactured by Tediore and comes from the Borderlands 2 Base Game.

Blockhead - Unique Ability

The Blockhead fires an expanding 3x3 Minecraft Fire Grid that explodes after two surface ricochets or upon enemy contact. The explosion deals 100% Fire Grenade Damage in an area and 100% Fire Bullet Damage on a direct hit.

‍The Blockhead is thrown and explodes on enemy contact or after 1.7s.
‍‍The Reload Explosion has a radius falloff and deals:
Fire Grenade Damage = Card Damage × Unused Ammo × 4.1*

*Factor: 0.5 + 0.4 × Pellets (min. 1.5)
